While commonly affecting the liver and lungs, rare localizations can pose diagnostic and therapeutic challenges. This report presents a case of CHD affecting the left temporal bone, originating from the mastoid cells, resulting in left transverse sinus obliteration and cerebellar hemisphere compression. This unusual presentation highlights the importance of considering CHD in the differential diagnosis of neurological symptoms, including vertigo, even in the absence of more typical clinical signs.
